BMI thresholds
BMI can help frame a medication assessment, but a number cannot determine treatment on its own. This guide explains common adult thresholds, measurement limits and questions for a clinician. Miami Weight Loss is an information and inquiry website.
The numbers
BMI expresses weight in relation to height: weight in kilograms divided by height in meters squared. For adult weight-management medication, common starting criteria are a BMI of at least 30, or at least 27 with a weight-related condition. The qualifying conditions guide explains why the diagnosis alongside the number matters. Meeting a commonly used threshold supports a clinical discussion, rather than confirming a prescription.
Use current, accurately recorded height and weight when discussing weight loss medication qualification. A clinician can check an unexpected calculation against the measurements. If you are close to a cutoff, report the actual figures instead of rounding them to a more favorable result. Ask which starting measurement and treatment criteria the provider is using so you understand the decision.
The purpose of the prescription also matters. Wegovy treatment criteria include different approved uses, and Zepbound eligibility includes a specific sleep-apnea indication. A general weight-management threshold does not describe every use of these medicines. The prescriber must consider the exact product, diagnosis, safety restrictions and applicable clinical requirements, rather than applying one number to every situation.
Working it out
Height without shoes, weight first thing. Small errors in height affect the result more than people expect, because it is squared.
Weight in kilograms divided by height in metres squared. Any calculator will do it, and the number is the same everywhere.
If you land between 27 and 30, whether you have a documented weight-related condition is the deciding factor.
A clinician checks the figure against your records, because self-reported height and weight drift.
Its limits
Two people can have the same BMI and different health profiles. The calculation does not separate muscle, fat and bone or show where fat is carried. Questions about muscle and body weight are therefore relevant when interpreting the number. A clinician may consider examination findings, blood pressure, laboratory results and medical history alongside it. The broader weight loss eligibility discussion should also consider what treatment is intended to accomplish. BMI is useful for screening, but it cannot supply that whole picture.
A person whose measurements fall outside common medication criteria can still seek advice about health, eating and activity. A medical weight loss program may address concerns beyond the decision to prescribe. Ask what support is appropriate and which measurements would be useful to follow over time. The weight loss timeline guide can help frame questions about reviewing progress. Changes in BMI are one part of that review; they should be considered with how you are doing and the clinician's assessment.

How to interpret a calculation without treating it as a diagnosis or prescribing decision.
Straightforward. Number was above the line, the conversation moved on to my history rather than dwelling on it.
Was sent for a sleep study before anything was decided. Turned out I did have apnea, which changed everything.
My height was out by three centimetres from what I had been telling people. Small change, noticeable difference in the number.
First time anyone admitted BMI is a blunt tool and told me what else they were looking at.

The number on its own rarely settles it. The free check looks at your BMI alongside the conditions and medications that actually decide the answer.
