Molecule comparison
These medicines differ in how they act, the products available and the evidence behind them. A useful comparison brings those differences together with your health, side effects and ongoing costs. The choice belongs in a clinical assessment.
The brand map
An ingredient name is only the beginning of a medication comparison. Semaglutide acts on GLP-1 receptors, while tirzepatide acts on both GIP and GLP-1 receptors. Both influence appetite and food intake. That difference in receptor activity does not tell a clinician everything needed to choose a treatment or establish an equivalent dose.
The finished product matters. Wegovy contains semaglutide and has injection and tablet formulations with their own instructions. Mounjaro contains tirzepatide and is used for type 2 diabetes. Products sharing an ingredient can differ in approved uses, formulation and administration. Ask for the actual brand or compounded preparation being proposed, rather than accepting an ingredient name as a complete description.
A clinical discussion also considers why treatment is being offered. Weight management, diabetes and other approved indications are not interchangeable. Your clinician should explain how the particular product fits your health needs and which precautions apply. Keep the formulation and intended use visible when checking a pharmacy label, reading a treatment quote or asking about a change. A similar name or number on a package does not make two prescriptions interchangeable.
How the choice is made
Some histories rule out one or both. Personal and family history, existing conditions and current medication all narrow the field before preference enters it.
Digestive side effects differ between people more than between molecules. Someone who cannot tolerate one may do well on the other, and switching is a normal step.
Price and what a drug plan will fund differ by product. A molecule you cannot afford to stay on is not the better option for you.
Supply of these products has been uneven. What is actually obtainable is part of the decision rather than a footnote to it.
What trials show
In SURMOUNT-5, a randomized, open-label trial lasting 72 weeks, adults with obesity and without type 2 diabetes received specified maximum tolerated weekly injection doses. Average weight reduction was greater with tirzepatide than with semaglutide. That finding applies to the treatments and population studied; it is not a comparison of every current formulation or dose. The weight loss timeline guide explains why study averages do not predict one person's progress. Both trial groups experienced digestive adverse effects, making common GLP-1 side effects part of the discussion.
For an individual decision, consider the expected benefit alongside medical history, other prescriptions and the practical demands of treatment. GLP-1 dosing and titration differ by product, and a higher number of milligrams does not establish greater effectiveness across different medicines. Ongoing weight loss medication costs also need checking for the exact prescription and payment arrangement. Tell the clinician about any difficulty taking, obtaining or paying for treatment. Those details can affect the plan, but they cannot be resolved by choosing whichever medicine has the larger trial average.

Brand names, formulations, switching and payment details that need an individual answer.
Started on one, could not tolerate it past the second step, moved to the other and it was fine. Treated as routine rather than a problem.
I came in convinced I needed the newer one. Ended up on the other for coverage reasons and it has worked. The reasoning was explained rather than asserted.
Honest that the trial difference is an average, not a promise. That framing was more useful than the numbers.
Finally understood that two of the four names I had been researching were the same drug.

The comparison only goes so far on a page. Which one fits you depends on what you take, what you have been diagnosed with, and what you can stay on.
